Tissue Plasminogen Activator also known as TPA is a drug prescribed to treat conditions caused by arterial blood clots such as heart attacks, thrombotic strokes, chest pain from unstable angina and pulmonary embolism

Dosage

Recommended treatment dose of Activase is 0.9mg/kg (not to exceed 90mg total treatment dose) infused over 60 minutes. 10% of total treatment dose should be administered as an intial bolus over 1 minute The remaining treatment dose should be infused intravenously over 60 minutes

Administration

  1. Get an accurate weight of the patient
  2. Calculate dosage for patient. See Dosage
  3. The medication will come in a 100mg vial. Discard excess
  4. Draw 10% from the total dose of the medication. You can either:
    1. Draw directly from the bottle
    2. Draw from the primed tubing
  5. Administer bolus dose over 1 minute
  6. Administer infusion dose over 60 minutes

Contraindication

Do not administer Activase to treat acute ischemic stroke in the following situations in which the risk of bleeding is greater than the potential benefit

  • current intracranial hemorrhage (ICH); subarachnoid hemorrhage;
  • active internal bleeding
  • recent (within 3 months) intracranial or intraspinal surgery or serious head trauma
  • presence of intracranial conditions that may increase the risk of bleeding (e.g., some neoplasms, arteriovenous malformations, or aneurysms
  • bleeding diathesis; and current severe uncontrolled hypertension.

Do not administer Activase to treat acute myocardial infarction or pulmonary embolism in the following situations in which the risk of bleeding is greater than the potential benefit:

  • active internal bleeding
  • history of recent stroke
  • recent (within 3 months) intracranial or intraspinal surgery or serious head trauma
  • presence of intracranial conditions that may increase the risk of bleeding
  • bleeding diathesis
  • and current severe uncontrolled hypertension.
